An AI scribe is a tool that captures the conversation between a clinician and a patient, either as a recording or in real time, and uses speech recognition and large language models to draft clinical documents such as visit notes and referral letters. Because it listens in the background, it is also called an ambient AI scribe. Its purpose is to reduce the time clinicians spend typing records during and after visits.

Drafts can contain transcription errors or omissions, so clinicians review, correct and sign off on them. The name comes from the medical scribe, a human assistant who documents visits on a physician's behalf.
